精子线粒体膜电位与供精妊娠结局的相关性研究
Relationship between sperm mitochondrial membrane potential and the outcome of artificial insemination with donor semen
目的 比较用于供精人工受精(artificial insemination with donor semen,AID)技术的人类精子库冷冻精液标本高生育力组、中生育力组与低生育力组精液常规参数、精子形态、精子线粒体膜电位水平,精子DNA碎片率。探讨上述指标与供精妊娠结局的相关性,从而筛选出评估人类精子库捐精志愿者生育力的有效指标。 方法 收集外供生殖中心用于AID同批次冷冻精液标本共140例,分别是“低生育力组”44例、“中生育力组”48例以及“高生育力组”48例。使用流式细胞仪检测精子线粒体膜电位水平(mitochondrial membrane potential,MMP)以及精子DNA碎片率(DNA fragmentation,DFI)。比较各组间精液常规参数、正常精子形态率、精子MMP以及精子DFI。 结果 年龄(P=0.476),禁欲天数(P=0.320)、体质指数(P=0.475)、精子浓度(P=0.751)、前向运动精子百分率(P=0.230)、正常精子形态率(P=0.444)在各组间差异无统计学意义,精子MMP(P=0.000)、精子DFI(P=0.004)在各组间存在显著性差异。AID临床妊娠率与精子DFI呈负相关(r=-0.308,P=0.000)。 结论 精子MMP、精子DFI是精子功能评价的重要指标。精子MMP是否能成为完全独立的精子质量指标尚有待进一步的研究。精子MMP联合精子DFI能否预测AID妊娠结局,还需进一步研究。
Objective To compare the routine semen parameters,sperm morphology,sperm mitochondrial membrane potential(MMP),and sperm DNA fragmentation index(DFI) of frozen semen specimens in the high-fertility,medium-fertility,and low-fertility groups from a human sperm bank used for artificial insemination with donor semen(AID),and to investigate the association of the above indicators with AID outcome to select useful indicators for assessing the fertility of sperm donors. Methods We collected 140 frozen semen specimens of the same batch used for AID from a reproductive center,including 44 specimens in the low-fertility group,48 specimens in the medium-fertility group,and 48 specimens in the high-fertility group. Sperm MMP and DFI were measured by flow cytometry. Routine semen parameters,the percentage of morphologically normal sperm,sperm MMP,and sperm DFI were compared between the groups. Results There were no significant differences in routine semen parameters and the percentage of morphologically normal sperm between the groups(P>0.05). Sperm MMP and DFI were significantly different between the groups(P<0.01). The clinical pregnancy rate of AID showed a significant negative correlation with sperm DFI(r=-0.313,P<0.01). Conclusion Sperm MMP and sperm DFI are important indicators for sperm function evaluation. Further studies are needed to determine whether sperm MMP can be a completely independent indicator of sperm quality and whether sperm MMP combined with DFI can predict pregnancy outcome in AID.
