正常成人电刺激痛感觉阈与耐痛阈相关性及影响因素的多元线性回归分析

摘要

目的 测定正常成人在电刺激下的痛感觉阈(perception pain threshold,PTH)、耐痛阈(pain tolerance,PTO)水平,探讨影响因素,分析痛感觉阈、耐痛阈的关系,并建立正常成人的PTH、PTO参考值范围。 方法 采用一元线性回归、多元线性回归分析等方法,回顾性分析2017年10月21日至2020年5月22日在陆军军医大学第一附属医院疼痛科使用体感诱发电位测量仪系统进行电刺激测试的正常成人痛阈相关数据,性别、年龄、职业、学历、婚姻、月收入、体质指数(body mass index,BMI)等一般资料。计算正常成人PTH、PTO参考值范围。 结果 一元线性回归分析显示PTH与PTO的变化呈高度正相关(P<0.001)。分别以PTH、PTO的值为因变量,将性别、年龄、职业、学历、婚姻、月收入、BMI指数为自变量进行多元线性回归分析,结果表明性别是PTH、PTO的影响因素(P<0.001),不同性别PTH、PTO值有明显差异。男性、女性PTH、PTO的值均呈正态分布(P均<0.05),采用正态分布法计算男性PTH、PTO的95%参考范围分别为0.051~4.388、1.845~13.012,女性PTH、PTO的95%参考范围分别为0.146~3.069、1.521~10.207;男性PTH、PTO的平均值±标准差分别为2.219±1.224,7.429±8.115,女性PTH、PTO的平均值±标准差分别为1.608±0.556,5.864±4.910。 结论 疼痛的敏感性女性高于男性,女性PTH、PTO正常值范围均较男性窄。疼痛的感受性与耐受性高度正相关。

Abstract

Objective To measure the levels of perception pain threshold(PTH) and pain tolerance(PTO) threshold in normal adults under electrical stimulation,to investigate related influencing factors and the correlation between PTH and PTO,and to establish the reference value ranges for PTH and PTO in normal adults. Methods Univariate linear regression analysis and multivariate linear regression analysis were used to perform a retrospective analysis of related data of the normal adults who underwent electrical stimulation tests using the somatosensory evoked potential measurement system in Department of Pain,The Southwest Hospital of Army Medical University,from October 21,2017 to May 22,2020,including pain threshold data,sex,age,occupation,education background,marriage,monthly income,and body mass index(BMI). The reference value ranges for PTH and PTO were calculated. Results The univariate linear regression analysis showed a highly positive correlation between PTH and PTO(P<0.001). With PTH and PTO as dependent variables,sex,age,occupation,education background,marriage,monthly income,and BMI were included in the multiple linear regression analysis as independent variables,and the results showed that sex was an influencing factor for both PTH and PTO(P<0.001),with significant differences in PTH and PTO between different sexes. PTH and PTO showed normal distribution in both male and female adults(P<0.05). The normal distribution method showed that the 95% reference ranges of PTH and PTO were 0.051-4.388 and 1.845-13.012,respectively,in male adults and were 0.146-3.069 and 1.521-10.207,respectively,in female adults; the mean±standard deviation values of PTH and PTO were 2.219±1.224 and 7.429±8.115,respectively,in male adults and were 1.608±0.556 and 5.864±4.910,respectively,in female adults. Conclusion Female adults are more sensitive to pain than their male counterparts,and the normal value ranges of PTH and PTO in female adults are narrower than those in make adults. There is a strong positive correlation between the perception and tolerance of pain.
